My best suggestion would be for you to write a fantasy book. A fantasy book will give you everything you need.
1. The Scene. Depending on the scene of your book, you make the landscape of your game. 2. The Plot. This will give you an overarching purpose for your leveling/quests/killing/grinding/ whatever. 3. Bestiary. This determines what kind of flora and fauna are in your environment and what creatures and plants show up in your book. They should match the scenery. Characters have to fit in somewhere. 4. Side stories. Rarely in a book does the protagonist just wander right to the end of the plot. There's usually side-ventures. These would be good quest/grind/kill spots for your MMO characters.
So before you make an MMO on a topic that you want us to pick, I suggest that you be sure you have the creativity to write a damn good book about it too.
